For those interested in creating a Living Will, The Minnesota Catholic Conference has prepared a Minnesota Catholic Heath Care Directive that meets the state's legal requirements and reflects the Church's teaching.

The National Catholic Bioethics Center offers a Catholic Guide to End-of-Life Decisions and an Explanation of Church Teaching on Advance Directives.
