Archbishop Participation Request Form
1. For event(s) taking place from July 1, 2025 through June 30, 2026, please click aquí to complete and submit your request form. Please submit one form per event request and be sure to complete all required form fields.
Once the request is submitted successfully, you will receive an email confirmation of your submission. (If you do not receive an email confirmation, your request may not have been received.)
Our April 4 deadline has passed for inclusion in the Annual Bishops’ Review at the end of April. New requests received will be reviewed following the Annual Bishops’ Review.

If your request is a Mass, and it has been confirmed by the Archbishop or Bishops’ Office(s), please complete the appropriate Mass Planning Document below:
- Archbishop Hebda: Please fill out this Mass Planning Document (PDF, Adobe Reader or Acrobat required) and return it to the Archbishop’s Office at zangsp@archspm.org o stevekenj@archspm.org. Please see the following ceremonial guide for your reference from our Office of Worship.
- Bishop Izen: Please fill out this Mass Planning Document (PDF, Adobe Reader or Acrobat required) and return it to Bishop Izen’s office at kaardala@archspm.org.
- Bishop Kenney: Please fill out this Mass Planning Document (PDF, Adobe Reader or Acrobat required) and return it to Bishop Kenney’s office at portillam@archspm.org.
Completed Mass planning documents should be emailed a minimum of two weeks prior to the Mass date.
