Archbishop Participation Request Form
- For events taking place from July 1, 2026, through June 30, 2027, please click here to complete and submit your request form. If this request is submitted by April 1, 2026 (the deadline for events occurring between July 1, 2026, and June 30, 2027), you will receive a response to your request following the Annual Bishops’ Review, beginning in May 2026. If your request is received after the deadline, it will be reviewed in the order received once the Annual Bishops’ Review has been completed.
- For events taking place prior to June 30, 2026, please click here to complete and submit your request form. Requests will be reviewed in the order received.
Please submit one form per event request and be sure to complete all required form fields.
Once the request is submitted successfully, you will receive an email confirmation of your submission. (If you do not receive an email confirmation, your request may not have been received.)
If your request is a Mass, and it has been confirmed by the Archbishop or Bishops’ Office(s), please complete the appropriate Mass Planning Document below:
- Archbishop Hebda: Please fill out this Mass Planning Document (PDF, Adobe Reader or Acrobat required) and return it to the Archbishop’s Office at zangsp@archspm.org or stevekenj@archspm.org.
- Bishop Izen: Please fill out this Mass Planning Document (PDF, Adobe Reader or Acrobat required) and return it to Bishop Izen’s office at kaardala@archspm.org.
- Bishop Kenney: Please fill out this Mass Planning Document (PDF, Adobe Reader or Acrobat required) and return it to Bishop Kenney’s office at portillam@archspm.org.
Completed Mass planning documents should be emailed a minimum of two weeks prior to the Mass date.
