Archbishop/Bishop Participation Request Form 2024-2025

For Events Taking Place July 1, 2024 – June 30, 2025:

Please use the form below to request participation of Archbishop Hebda, Bishop Williams, and/or Bishop Izen in your event. This form should be used only for events taking place July 1, 2024 – June 30, 2025. Please fill out one form per event request. Please be sure all required form fields are completed.

Once the request is submitted successfully, you will receive an email confirmation of your submission. (If you do not receive an email confirmation, your request may not have been received.)

Requests will be reviewed after the annual Bishops’ Review in the order received, beginning May 2024.
